Frequently Asked Questions

Can I fax enrolment or change forms?
Yes, the enrolment form can be faxed for processing but please mail the original for our files. You will be provided with contact information including the fax number.

Does the employee need to sign an enrolment or change form?
Yes, the employee signature is required on all enrolment or change forms.

Once an enrolment or change form is submitted, how long does it take before I receive a new wallet card?
It will take approximately 15 working days for a new wallet card to arrive.

How do I get more employee benefit plan booklets if I need them?
You will be assigned an Account Manager. You will be able to reach them by email, phone or fax.

How do I make changes to employee information?
It depends on the type of change. If you are unsure of which administration form to complete, please contact your Account Manager.

How do I report an employee termination?
You will need to complete a Report of Employee Transactions form. This can be faxed or mailed; or we can accept notification by e-mail containing the name of the employee and their date of termination.

When is health evidence required?
Health evidence will be required for any employee applying for coverage more than 31 days after the effective date of coverage.

Is there coverage for dependents of a deceased employee?
Survivor Benefits are available. Please contact your Account Manager for further details.

What if an employee dies or becomes disabled?
In the event of a Life, Accidental Death & Dismemberment claim, please contact your Account Manager that has been assigned to you.

Do I need to sign Extended Health and Dental claim forms?
No.

When are invoices issued?
Invoices will be sent between the 15th and the 20th of the month. Your payment is due on the first of the following month.

Can I use pre-authorized chequing to pay my monthly bill?
Yes. Necessary forms will be provided. 

Is short term disability and long term disability coverage available?
If you are interested in short term and long term disability you can ask Mark Beckham for information on individual coverage.

Glossary of Terms

Accidental Death & Dismemberment
AD&D provides financial assistance if tragedy strikes and the plan member dies as the result of an accident. It also provides coverage if an accident results in the loss of a limb, sight or hearing. Some additional benefits include rehabilitation, spousal occupation re-training and a daycare benefit.

Critical Illness
Critical Illness provides a lump sum benefit payable to an employee who survives for 30 days (or 180 days for paralysis, MS or loss of speech) after a covered illness or injury has occurred. It provides financial resources that allow the employee to adjust to the changes in their lifestyle that will result after having suffered a critical illness or injury.

Dental Care
Dental Care plans vary by the extent of the services being covered and your specific plan design. Dental services can be broken down into three categories: Basic Services (cleanings, fillings, extractions), Major Restorative Services (dentures, crowns, bridges) and Orthodontic Services.

Dependent Life
Dependent Life provides a life insurance benefit for the plan member's spouse and dependent children.

Extended Health Care
Health care plans provide a wide range of coverage to supplement coverage provided by provincial health care plans, including Drug Plans, Vision Care and Out of Province Coverage.

Life Insurance
Group life insurance provides financial assistance in the event of a plan member's death. Optional Life insurance is also available.
