Group Conversion
On termination of your employment, you may be eligible to convert the following Group Benefits to individual plan(s) without health evidence.
- Life Insurance, Accidental Death & Dismemberment benefit
- Extended Health and Dental benefit
- Critical Illness (subject to continuous coverage for 24 months)
Policies will be issued only if application is made within certain time limits:
- 31 days after your termination date for Life, AD&D and Critical Illness
- 60 days after your termination date for Extended Health and Dental
