Where can I get forms?
All forms can be downloaded from the Documents page
on the Plan Administrator menu, or you may contact your BBD
client service representative.
Can I fax enrollment or change forms to BBD?
Yes, the enrollment form can be faxed for processing but
please mail the original, as it is required for our files.
Does the employee need to sign an enrollment or change form?
Yes – the employee signature is required on all enrollment
or change forms.
Once an enrollment or change form is submitted, how long does it take before I receive a new wallet card?
It will take approximately 15 working days for the new
wallet cards to arrive.
How do I get more employee benefit plan booklets if I need them?
Contact your Account Manager at BBD.
How do I make changes to employee information?
It depends on the type of change. If you are unsure of
which administration form to complete, please contact your
BBD client service representative.
How do I report an employee termination?
You will need to complete a Report of Employee Transactions
form. This can be faxed or mailed; or we can accept notification
by e-mail containing the name of the employee and their
date of termination.
When is health evidence required?
Health evidence will be required for any employee that is applying for coverage more than 31 days after the effective date, or when an employee is eligible for coverage in excess of the non-medical maximum under Life and/or Long Term Disability.
Is there coverage for dependents of a deceased employee?
Survivor Benefits are available. Please contact your Account
Manager for further details.
What if an employee dies or becomes disabled?
In the event of a Life, AD&D, Short Term Disability
or Long Term Disability claim, please contact your Account
Manager.
Do I need to sign Extended Health and Dental claim forms?
No.
When are invoices issued?
Mid-month – between 15 – 20th of the month.
Your payment is due on the first of the following month.
Can I use pre-authorized chequing to pay my monthly bill?
Yes.
