Group Administrators must use this form to authorize his or her Plan Advisor to allow access to their Group's Employee Information. The Plan Advisor will use this information to assist in determining the Employee's benefit needs.
Use this form if an employee has a dependent child who has reached your plan's age limit (i.e. age 21), and is attending school full time or is handicapped.
